Referral Intake Form

From 1st July 2016 the way Hunter Prelude receives funding to support children with a disability or development delay has changed. Funding to support children will now come from the National Disability Insurance Scheme (NDIS). We are currently waiting to hear how the NDIS will fund new children who are referred to our service that do not have an NDIS Plan. As the NDIS is in the transitional phase the most up-to-date information is available via their website www.ndis.gov.au or by phoning 1800 800 110.

Have a plan already…
Hunter Prelude is a registered service provider of Early Childhood Supports and/or Therapy supports under the NDIS.

Hunter Prelude also provides support for children with “Helping Children with Autism” and “Better Start for children with Disability” funding.
